Our anticoagulation clinics provide education, blood testing, and dosing instructions for patients taking blood thinners. Our staff will work closely with other members of your healthcare team to ensure that all your medical needs are addressed. Studies have shown that patients enrolled in an anticoagulation clinic have better international normalized ratio (INR) control, better outcomes, and fewer problems. Dizziness is one of the most common reasons for a visit to your doctor, but it is not a very descriptive term. It may include other symptoms of a balance disorder, including feeling lightheaded, heavy-headed, faint, disoriented, a spinning sensation (vertigo), and the inability to place oneself in three-dimensional space. Dizziness is a warning signal sent by the body to tell us that something is not functioning properly.
For the body to feel balanced, the brain requires input from the inner ear, eyes, muscles, and joints. Since mechanisms for maintaining balance are so complex, finding the exact cause of dizziness is often difficult and requires input from several medical specialties. Dizziness can result from problems associated with the inner ear, brain, or heart.

When dizziness is followed by fainting or a loss of consciousness, you need prompt medical attention. When dizziness is accompanied by symptoms such as weakness of an arm or leg, double vision, slurred speech, clumsiness of just one arm or leg, facial numbness, or facial weakness, seek immediate care at the nearest ER, as these are signs of stroke.
The chronic care management team at CPC is tailored specifically to care for patients who live with complex medical conditions. We have a team of physicians, nurses, and nurse coordinators who are experienced in and dedicated to the care of patients with conditions such as diabetes, heart failure, hypertension, chronic obstructive pulmonary disease (COPD), and more. Our goal is to serve as a medical home for patients with multiple medical needs and to manage their care in such a way that their quality of life is improved and the day-to-day workload of managing their illnesses is reduced. We manage medications, diagnostic testing and screenings, and coordinate with specialists and community resources such as home health agencies.
The CPC on-site clinical laboratory accepts specimens and performs lab work for our physicians and referring physicians. The lab is staffed with experienced phlebotomists and clinical laboratory scientists.

Our transitional care program is designed to fill a gap in healthcare oversight. CPC family medicine physicians help patients manage the transition from an inpatient facility to a residential or community setting.
